HSE Guidance L65 – Third edition

Prevention of fire and explosion, and emergency response on offshore installations.

In this third edition, the ACOP text and associated guidance have been updated and simplified, where appropriate. The main changes have been widely consulted on and include:

emphasising when developing an approach to fire and explosion hazard management, consideration is given to ‘timely detection’ of occurred events and the ‘appropriate reporting and recording of them’; when undertaking a regulation 5 assessment, clarification is given that releases of toxic or asphyxiating gases with the potential to require evacuation, escape and rescue are included. Also, guidance is given on considerations associated with the provision of totally enclosed motor-propelled survival craft; guidance covering when a repeat assessment could be required when changes including improvements are introduced following the initial assessment; and a clearer definition of a ‘performance standard’; adding suitable detection systems for asphyxiating atmospheres to the list of illustrative detection arrangements and providing a further explanation of detection systems, alarm settings and their capabilities; the temporary refuge survival time should be adequately defined in the emergency response plan and events likely to compromise temporary refuge integrity need to be identified and addressed.

Yorkshire Water fined £1.1million

Yorkshire Water has been fined £1.1million for illegally discharging sewage that polluted the River Ouse near York. The sewage had overflowed into the river because of a pump failure at the treatment works.

At the time of the incident, when one of the pumps failed, the backup was not operational. It has been out of use for five months – a breach of the firm’s environmental permit.

With only two pumps working, sewage flowed into emergency storage tanks, filled them up, and approximately 6,000 cubic metres of sewage overflowed through an old outfall into the river, at a location where discharges are not permitted.

Derbyshire firms prosecuted for hand arm vibration offences

Two Derbyshire-based manufacturing firms were fined a total of £290,000 and ordered to pay £16,000 costs for safety breaches resulting in cases of carpel tunnel syndrome and hand arm vibration syndrome to their employees. It was reported at Derby Crown Court that the employees were regularly exposed to hand arm vibration through the use of a range of vibratory tools in the assembly and servicing of crushers and screeners.

ECHA consultation

The European Chemical Agency (ECHA) has ECHA has launched a public consultation on applications for authorisation covering uses of Chromium VI compounds; used in surface treatment, hard chrome plating, conversion coating, passivation, cooling systems, 1,2-dichloroethane (EDC) used as solvent and as a swelling agent, Bis(2-methoxyethyl) ether (Diglyme); Used as a carrier solvent and processing and Formaldehyde, oligomeric reaction products with aniline; used in an epoxy resin hardener.
