HSE has issued a safety notice after identifying high concentrations of hydrogen sulphide

HSE has issued a safety notice after identifying high concentrations of hydrogen sulphide (H2S) in ullage spaces of cargo and slop tanks.  Duty holders operating floating production storage and offloading (FPSOs) and floating storage units (FSUs) must regularly monitor ullage spaces of cargo and slop tanks for the presence of toxic gas such as hydrogen sulphide, using appropriate equipment. If detected, duty holders must ensure that the installation has adopted and implemented a suitable hydrogen sulphide management process in accordance with HSE’s information sheet, Managing hydrogen sulphide detection offshore.

HSE issue Safety Notice with respect to LPG powered fork lift trucks

The Health and Safety Executive has issued a Safety Notice with respect to fires on, and around, liquefied petroleum gas (LPG) powered forklift trucks and other similar vehicles which have occurred during start up. The problem has been caused by a build-up of deposits in vaporiser units, flow regulators and shut off valves which cause the truck to fail to start, particularly from cold, leading to sticking valves and the mixture supplied to the engine being too rich to be ignited. Repeated starting attempts can result in the release of unburned LPG, which in extreme cases can be ignited, setting fire to the vehicle and surrounding materials.  https://www.hse.gov.uk/safetybulletins/forklift-fires.htm

Cell phone use results in $12 million collision

The National Transportation Safety Board (NTSB) has issued a report of an investigation into the collision between a bulk carrier and an offshore supply vessel. The incident occurred on 23rd July 2022 in the Gulf of Mexico. The collision between the carrier, Bunun Queen, and the supply vessel, Thunde,r resulted in damage to both vessels estimated at $12.3 million. The NTSB determined that the probable cause of the collision was Bunun Queen watch officer’s distraction due to performing “non navigational tasks” and the Thunder officer’s distraction due to cell phone use.  These activities kept both officers from keeping a proper lookout. https://www.ntsb.gov/investigations/Pages/DCA22FM030.aspx

Failure to carry out tests following maintenance results in rail accident

An American freight train derailed following the seizure of an axle on one of the cars.  Hot axle box detection systems had been fitted to the tracks but following maintenance they had been reconnected incorrectly.
